Please attach recent x-rays, photos needed will be one smiling straight on and another smiling with teeth slightly apart straight on.
We require an initial payment of $150.00
$150.00 is non-refundable. However, if you decide to schedule an appointment with Dr. Torres the $150.00 will be applied towards your in office consultation fee.
